Scream writer Kevin Williamson released his first slasher in years with 2022's, Sick. It follows friends Parker and Miri as their plans to quarantine together are thwarted by a masked intruder out for blood. So, what happens in the Sick movie ending?

There are spoilers, of course, for the film Sick. This list will break down the chaotic events of the film’s final act, as well as explain who was the “big bad,” and why. If you had any lingering questions after watching Sick, you should find your answers below.

Read on to learn more about the Sick movie ending. You can also check out my review of the film, here. Then, you can always revisit the film - from director John Hyams (Alone, 2020) - which is streaming on Peacock.

There were three killers all along.

DJ (Dylan Sprayberry) figures out that there is someone in the house, but what he didn't realize, is that there were actually three people casing the place. Parker (Gideon Adlon) takes down one in the kitchen, with an assist from Miri (Beth Million).

The girls think they only have one person left to outrun, which is why Parker is relieved to see Pamela (Jane Adams) drive by as she's wrestling off Jason (Marc Menchaca). That relief does not last for long.

The 'Good Samaritan' Parker finds on the street is in on it.

After running from the neighbor's house, and fighting on the side of the road, Parker thinks she might finally be saved, when she spots a lone woman, Pamela, driving on the road. Comically, Pamela won't let Parker into the car without a mask, which is fair, but the young woman is being chased by a knife-wielding maniac trying to kill her.

Pamela gives Parker an “extra mask” she has in the car, and allows her to get inside. That is the horrible moment when the audience finds out this was all staged, and the woman has given Parker a mask laced with Chloroform, which she doesn't realize until the damage has already been done.

It all comes back to Benji.

Throughout the film, viewers are repeatedly shown a video of Parker kissing a boy named Benji at a party, to the tune of “A Little Respect” by Erasure. First, her friend Miri brings it up, to razz Parker about the fact that she probably only did it to make former flame DJ jealous. DJ brings it up when he and Parker are alone, for the same reason.

Later, when the attack first begins, Parker is woken up by the same Erasure song blasting through the house. She thinks DJ turned it on, but the attackers did, and for a very specific reason. Turns out, this whole thing has been about Benji.

He was the youngest son of Jason and Pamela, and he contracted COVID-19 after swapping saliva with Parker at that pre-quarantine college party. Sadly, that leads to him dying alone in a hospital after the virus took hold, because as you may remember, many hospitals instituted “no visitors” rules because of COVID-19. All of the violence and murder has been Benji's family's way of getting revenge for the loss of their son.

Did Parker have COVID-19?

Technically, yes, she did. However, she was asymptomatic, so she didn't realize she had the virus, and therefore had no idea she was infecting those around her - seven people from the party, including Benji. Jason and Pamela even did the tracing, and found the man who gave the virus to her, Tyler Murphy (Joel Courtney). Tyler is the man audiences see killed in the opening sequence of Sick.

Sick plays on both horror movie scares and real-life fears, as during the beginning of the pandemic, many were truly afraid of the notion that you can not only have COVID-19 without knowing it, but you can also spread it to other people without any hint of symptoms yourself.

What about Miri and DJ?

Pamela tells Parker that, “per CDC guidelines, you were meant to quarantine alone,” so it's her fault for putting them in harm's way. Unsurprisingly, they're not quite as self-reflective when it comes to the fact that their other son is now dead because they enlisted him to help them avenge Benji's death. DJ was simply in the wrong place at the wrong time, but he did die a hero.

How did Jason and Pamela think they'd get away with this?

Parker points out that between fingerprints, and DNA, the police will find her and Jason. Pamela says that “Irresponsible girls can cause irresponsible fires that burn down the house. Fires happen all the time. It's tragic.”

Ironically, it is later a fire that snuffs out the last of Pamela's fight.

So, what happens to Parker and Miri?

Parker is brought back to her family's lake house with Jason and Pamela, and forcibly given a COVID test. They taunt her, with Jason sporadically pulling a bag over Parker's head to suffocate her, while Pamela says, “How does it feel to not know when you're going to get your last breath? To feel like you're going to die, scared and alone?”

While Parker has pulled attention outside of the house, Miri resourcefully makes herself a splint for her broken leg. After a struggle with the man in the kitchen, Miri ultimately gives him a fatal stab wound. Then, she hides, attempting to dial 9-1-1 via her laptop, before Jason realizes that someone is using the wifi. When he goes outside to see if Miri is still there, she sneaks up behind Pamela and knocks her unconscious. Then, Parker pushes Pamela out the window.

While Jason is walking through the house, searching for the girls, Miri surprises him by cutting his Achilles' heel. Then Parker pushes him over the balcony, and he falls to his death, getting impaled on decorative deer antlers. Parker and Miri stumble to a nearby barn, hoping to find a new method of transportation, when Pamela comes running at them.

Pamela gets covered in fuel during her scuffle with Parker, and Miri finally puts an end to things by setting her ablaze.
